By using the services provided by Spark Wellness, you agree to the following terms and conditions:
1. Services
Spark Wellness provides mental health and substance abuse treatment, including counseling, education, medication monitoring, and support programs. Services are offered both in-person and virtually, based on client needs and clinical recommendations.
2. Appointments & Cancellations
Clients must give at least 24-hour notice to cancel or reschedule appointments. Failure to do so will result in a $40 missed appointment fee. Repeated cancellations may result in service restrictions.
3. Payment & Insurance
- Payment is due at the time of service unless prior insurance arrangements have been made.
- We accept cash, checks, and major credit cards.
- Clients are responsible for any unpaid balances not covered by insurance.
4. Confidentiality
All client information is kept strictly confidential and managed in accordance with HIPAA regulations. Please refer to our Privacy Policy for details.
5. Emergencies
Spark Wellness is not a crisis center. In the event of an emergency, call 911 or visit the nearest emergency room. Scheduled sessions are not substitutes for emergency care.
6. Code of Conduct
We reserve the right to refuse or discontinue services to any individual who engages in disruptive, abusive, or unsafe behavior toward staff or other clients.
7. Amendments
Spark Wellness may update these terms at any time. Continued use of our services indicates your acceptance of any changes.
